ADOPTION RULES

BLESS THEIR HEARTS KITTY HAVEN IA A NO-KILL SHELTER solely dedicated to saving the lives and providing a loving, safe, healthy and happy home to cats that are stray, have been abandoned, are no longer waited by their human family, or are sick.

We are seeking and expect the Adopter to provide a loving, safe, happy and healthy home that these kitties deserve. We expect our Adopters to not only provide the above but also provide the necessary medical care for the rest of the kitties natural life (12 or more years).

TERMS OF ADOPTION

   * All Adoption Applicants must be interviewed and approved
     The interview will be conducted by the director

   * Must be 18 years or older to adopt

   * Must have valid picture identification with name and current address on it

   * All members of the family living in home must be present at time of
     adoption

   * If single and living at home, you must have signed consent of parents
     and one parent must be present at time of adoption

   * Under no circumstances is this cat to sold or given away to another
     person, pet shop or research facility

   * If for any reason the new parents can no longer take care of the cat, the
     cat MUST be returned to Bless Their Hearts Kitty Haven

   * All cats must be spayed or neutered

   * Cats must not be allowed outdoors for any reason

   * The cat will NOT be declawed

   * Following adoption, we will make followup visits and phone calls for a period
     of 1 year

   * If the cat is ever found to be in ill health (with no medical treatment) or is
     in a dangerous or negligent situation, the cat will be forfeited or returned to
     us

   * Cats will not be punished by hitting with any object or locked up in
     small rooms, garages, etc.

   * Cats will not be placed in a home where other cats have not been
     spayed/neutered, been tested for FVRC and Felv and are not up to date
     on their immunizations

We realize that our guidelines are strict, but we want to provide the best home and family for our kittys. If our potential Adopters are not willing to follow our guidelines, we will care for the kittys ourselves.

Adoption Contract

I ________________________________________ am adopting the Cat(s) listed here as my permanent INDOOR-ONLY family pet(s).  I agree to provide yearly veterinary exams, vaccinations and any necessary care for the life of the cat.  If adopting an unaltered kitten, I agree to comply with Bless Their Hearts Kitty Haven, Inc. spay/neuter program. In addition, I agree to not declaw the cat(s). If, for any reason during the cat’s lifetime, I can no longer properly care for or provide a quality permanent home for the cat(s), I agree to contact Bless Their Hearts Kitty Haven, Inc. (1) for assistance in placing  the cat into another appropriate home or (2) to return the cat directly to Bless Their Hearts Kitty Haven, Inc.

I understand that the cat or kitten I am adopting has been rescued.  It may have come to Bless Their Hearts Kitty Haven, Inc. from A Duval County Animal Control shelter, it may have been given up by a former owner or perhaps it was abandoned or strayed.  While in Bless Their Hearts Kitty Haven, Inc. care, the cat/kitten has been provided appropriate medical care as needed.  There is however, a chance that my new cat/kitten could have been exposed to a virus, such as upper respiratory infection or other illnesses, while on its way to being rescued and adopted. I agree to be financially responsible for veterinary costs, if any, associated with continuing care. I agree to obey all local laws regarding pet ownership, including care, feeding, sheltering and licensing of my  new cat(s). I understand I am assuming any and all risks associated with this adoption.  I further understand that if it harms, injures or makes ill any person or other animal, or damages any property, I shall assume all responsibility for such, and that Bless Their Hearts Kitty Haven, Inc. shall be held in any way responsible, obligated or culpable.

 I also agree for a staff member of Bless Their Hearts Kitty Haven, Inc. to periodically check on the welfare of the adopted cat(s) for a period of at least 1 year from the adoption date.

Your adoption fee/donation helps to defray the cost of caring for your new cat while it was waiting for you to adopt it. We are happy to work with you to ensure that this is the right care for you, but we are unable to issue a refund if the cat is returned for any reason.
